Common medical uses:
Cefuroxime axetil is a cephalosporin antibiotic. Cefuroxime axetil is used for the treatment of infections of lower respiratory tract, urinary tract, skin and skin structures; treatment of uncomplicated gonorrhea, otitis media, pharyngitis, and tonsillitis caused by susceptible strains of specific microorganisms. Treatment of early Lyme disease, pharyngitis/tonsillitis, and impetigo.

Cefuroxime axetil dosage:

For children 3 months to 12 years:

  • Usual dosage range:
    20-30 mg/kg/day in 2 divided doses
  • Sinusitis, otitis media, and impetigo:
    Suspension: 30 mg/kg/day in 2 divided doses for 10 days (maximum dose: 1 g/day)
    Tablet: 250 mg twice daily for 10 days
  • Epiglottitis:
    150 mg/kg/day in 3 divided doses for 7-10 days
  • Pharyngitis/tonsillitis:
    Suspension: 20 mg/kg/day (maximum: 500 mg/day) in 2 divided doses for 10 days
    Tablet: 125 mg every 12 hours for 10 days

For children from 13 years and adults:

  • Usual dosage range:
    250-500 mg twice daily
  • Bronchitis (acute and exacerbations of chronic bronchitis):
    250-500 mg every 12 hours for 10 days
  • Gonorrhea:
    1 g as a single dose
  • Lyme disease (early):
    500 mg twice daily for 20 days
  • Pharyngitis/tonsillitis and sinusitis:
    250 mg twice daily for 10 days
  • Skin/skin structure infection (uncomplicated):
    250-500 mg every 12 hours for 10 days
  • Urinary tract infection (uncomplicated):
    125-250 mg every 12 hours for 7-10 days

Cefuroxime axetil side effects:

  • Gastrointestinal: Nausea; vomiting; diarrhea; anorexia; abdominal pain or cramps; flatulence; colitis, including pseudomembranous colitis.
  • Genitourinary: pyuria; renal dysfunction; dysuria; reversible interstitial nephritis; hematuria; toxic nephropathy.
  • Hematologic: eosinophilia; neutropenia; lymphocytosis; leukocytosis; thrombocytopenia; decreased platelet function; anemia; aplastic anemia; hemorrhage.
  • Hepatic: hepatic dysfunction; abnormal LFT results
  • Miscellaneous: hypersensitivity, including Stevens-Johnson syndrome, erythema multiforme, toxic epidermal necrolysis; candidal overgrowth; serum sickness–like reactions (eg, skin rashes, polyarthritis, arthralgia, fever); phlebitis, thrombophlebitis, and pain at injection site.

Precautions:

Contraindications: Hypersensitivity to cefuroxime, any component of the formulation, or other cephalosporins.

  • Elevated INR: May be associated with increased INR, especially in nutritionally-deficient patients, prolonged treatment, hepatic or renal disease.
  • Penicillin allergy: Use with caution in patients with a history of penicillin allergy, especially IgE-mediated reactions (eg, anaphylaxis, angioedema, urticaria).
  • Superinfection: Prolonged use may result in fungal or bacterial superinfection, including C. difficile-associated diarrhea (CDAD) and pseudomembranous colitis; CDAD has been observed >2 months postantibiotic treatment.
  • Renal impairment: Use with caution in patients with renal impairment; modify dosage in severe impairment.
  • Suspension/tablet bioequivalence: Tablets and oral suspension are not bioequivalent; do not substitute on a mg-per-mg basis.

Drug interactions

  • Antacids: May decrease the effects of oral cefuroxime due to reduced absorption; separate dosing by 2 hours.
  • H2 antagonists: May decrease the effects of oral cefuroxime due to reduced absorption; separate dosing by 2 hours.
  • Probenecid: May increase the effects of cefuroxime.
  • Typhoid vaccine: Antibiotics may decrease the therapeutic effects of typhoid (Ty21a strain) vaccine; vaccination should be delayed >24 hours after administration of antibiotic.

Pregnancy & Lactation:

Animal studies have not shown any teratological effects.

Breast-Feeding:

Theoretically, drug absorbed by nursing infant may change bowel flora or affect fever work-up result.
As a class, cephalosporins are used to treat infections in infants.
