Medical uses:
Amoxicillin (Amoxil) is an aminopenicillin derivative with comparable gram-positive and gram-negative coverage to ampicillin. Amoxicillin is used for the treatment of otitis media, sinusitis, respiratory tract infections, skin infections, urinary tract infections.

    Mild to moderate infections: 25mg/kg/day in divided doses every 12 h or 20mg/kg/day in divided doses every 8 h.
    Severe infections: 45 mg/kg/day in divided doses every 12 h or 40 mg/kg/day in divided doses every 8 h.
  • Lower Respiratory Tract Infections
    Adults and children weighing at least 40 kg: 875 mg every 12 h or 500 mg every 8 h.
    Children (older than 3mo of age and weighing less than 40 kg): 45mg/kg/day in divided doses every 12 h or 40 mg/kg/day in divided doses every 8h.
  • Gonorrhea
    Adults: 3 g as a single dose.
    Prepubertal Children (2 yr of age and older): 50 mg/kg amoxicillin combined with 25 mg/kg probenecid as a single dose.

It is important to finish your entire antibiotic prescription, even if you fill better. Generally, you begin feeling better when the most of bacteria has been killed. Amoxicillin side effects:

  • CNS: Dizziness; fatigue; insomnia; reversible hyperactivity
  • Dermatologic: Urticaria; maculopapular to exfoliative dermatitis; vesicular eruptions; erythema multiforme; skin rashes.
  • Gastrointestinal: Gastritis; anorexia; nausea; vomiting; abdominal pain or cramps; epigastric distress; diarrhea or bloody diarrhea; rectal bleeding; flatulence; enterocolitis; pseudomembranous colitis.
  • Genitourinary: Interstitial nephritis (eg, oliguria, proteinuria, hematuria, hyaline casts, pyuria); nephropathy; vaginitis.
  • Hematologic: Anemia; hemolytic anemia; thrombocytopenia; thrombocytopenic purpura; eosinophilia; leukopenia; granulocytopenia; neutropenia; bone marrow depression; agranulocytosis; reduced hemoglobin or hematocrit; prolonged bleeding and prothrombin time; increased or decreased lymphocyte count; increased monocytes, basophils, platelets.
  • Hepatic: transient hepatitis; cholestatic jaundice.
  • Metabolic: elevated serum alkaline phosphatase and hypernatremia; reduced serum potassium, albumin, total proteins, and uric acid.

Precautions:

  • Hypersensitivity: Serious and occasionally severe or fatal hypersensitivity (anaphylactoid) reactions have been reported in patients on penicillin therapy, especially with a history of beta-lactam hypersensitivity, history of sensitivity to multiple allergens, or previous IgE-mediated reactions (eg, anaphylaxis, angioedema, urticaria). It is appropriate prescription only for people without a history of allergic reactions to similar antibiotics such as penicillins and cephalosporins.
  • Superinfection: Prolonged use may result in fungal or bacterial superinfection, including C. difficile-associated diarrhea and pseudomembranous colitis.

Amoxicillin drug interactions

  • Allopurinol: may increase the possibility of skin rash.
  • Oral contraceptives: Anecdotal reports suggesting decreased contraceptive efficacy with penicillins have been refuted by more rigorous scientific and clinical data.
  • Tetracyclines: may impair bactericidal effects of amoxicillin.
  • Laboratory Test interactions: may cause false-positive urine glucose test results with Benedict solution, Fehling solution, or Clinitest tablets (enzyme-based tests, eg, Clinistix , Tes-Tape , are recommended); false-positive direct Coombs test result in certain patient groups; false-positive protein reactions with sulfosalicylic acid and boiling test, acetic acid test, biuret reaction and nitric acid test (bromphenol blue test, Multi-Stix , is recommended).

Pregnancy & Lactation

Animal studies with amoxicillin have not shown any teratological effects. The product has been used extensively for more than twenty years and its suitability in human pregnancy has been well documented. When antibiotic therapy is required during pregnancy, amoxicillin may be considered appropriate. It may be used to treat chlamydia infections in pregnant women.

Breast-Feeding:
Amoxicillin is distributed into breast milk in low concentrations and should be used with caution in nursing women.

Overdose:
Symptoms of penicillin overdose include neuromuscular hypersensitivity (eg, agitation, hallucinations, asterixis, encephalopathy, confusion, and seizures). Interstitial nephritis and/or crystalluria, possibly resulting in renal failure, may occur; hydration and diuresis may be beneficial.
